AI Scribe for Physical Therapy: One PT’s Honest Take After 52 Years

Elaine Lu has been a physical therapist for over 50 years. She’s 79, she works in complex rehabilitation and assistive technology, and last fall she contacted Jane’s support team because she understood AI exactly zero percent.

Using AI Scribe to help manage rejections

Managing rejections from insurance companies, and justifying the choices she’s previously made, are big parts of Elaine’s business. In her line of work, she’s helping get patients assistive equipment like wheelchairs, but each piece of the wheelchair must be justified and accounted for in order for the insurer to reimburse the patient.

One of Elaine’s primary goals is to use Jane’s HIPAA and PIPEDA-compliant AI Scribe to decrease rejections. “I’m getting more approvals,” Elaine explained.

Essentially, Elaine is using AI to be sure the words she uses promotes approval. With the help of AI Scribe she’s speaking the insurer’s language and catching pitfalls that could cause rejections before they happen.

“But there are some, that no matter what you say their first time through, they automatically deny it. So then you have to do an appeal. And that has been where AI has been really good because instead of a whole new evaluation, I do most of that in a [dictation] narrative.”

Elaine’s process is time consuming, because it has to be. There’s travel to the evaluation location, and she added, “I’m in Utah, so snow is a factor.”

Then there’s the coordination of several schedules; hers, the patient’s, the patient caregiver, the supplier’s. Finally there’s the documentation time, which has taken her historically around four hours per patient. And as practitioners know all too well, all that admin is unpaid.

This is all of the work before and after an appointment, without taking into consideration the time to do the evaluation itself, which typically ranges from two to three hours, Elaine says. “I’m only doing probably four to six evaluations a week right now, but at two to three hours a piece, and that doesn’t include the prep, getting them scheduled, you know, that’s about all I can do.”

When Elaine first got in touch with Jane, her documentation process was 24 pages long. A few Zoom calls later and it’s down to eight.

Beyond the process improvements, Elaine is also understanding AI much better. When she first contacted support she claimed she understood AI exactly zero percent. Now? “Oh, 80 to 90%,” she says. “But we still need to do some tweaking.”
