SOAP Notes: How to Spend Less Time Charting with Jane and AI

“I [now have time to] eat my lunch and use the washroom so I can be more present with people,” says Sean Overin.

Sean, a physiotherapist, Jane Ambassador, and Director of Learning at Tall Tree Health is talking about some of the changes he’s noticed since adopting Jane’s AI Scribe into his workflow.

Some call it charting, others call it documentation, but it’s fair to say that no matter the name, taking great notes can take just a little too long.

This is the ethos behind many of Jane’s features, but especially for charting and documentation. Jane wants to help you provide excellent care, take great SOAP notes, and get to eat lunch.

It’s hard to talk about doing things faster without mentioning AI. This new tech evolution can genuinely change how clinics chart. Jane’s AI Scribe was built to help practitioners take thorough clinical notes in less time, all while maintaining HIPAA and PIPEDA compliance.

AI Scribe is a Smart Note chart part that can be added to any template (yes, even the one you currently use every day) that transcribes your sessions. You just add the chart part, customize the SOAP note prompt if needed, and record.

Once a session is finished, all you have to do is review your notes, make any necessary edits, and sign off.

Since its release in August 2025, almost 8,500 practitioners have used Jane’s AI Scribe to support their note-taking and documentation creation, which is exciting. 1

That said, recording a session isn’t always the best option for some practitioners.

Using AI for clinical notes without recording: try dictation

With this in mind, AI Scribe can also be used for dictation. After your appointment, or whenever you like to catch up on your notes for the day, you can speak your notes aloud and Jane will turn them into structured documentation for you. This helps save a little typing and time, without having to record during a session.

From a care perspective, Sean feels AI Scribe has made a big difference in his practice, improving his presence during an appointment and his workflow afterwards. “It allows me to focus on our conversation much more, and then it also allows me to communicate with [a patient’s] doctor a lot more efficiently if that’s needed, or any other allied health [professional].”

How Phrases can decrease repetition

If AI Scribe isn’t for you, no problem. Jane’s Phrases feature might be a better fit. It allows you to quickly and accurately write an entire chart entry using the sentences you type every day, without typing it out every time.

To start auto-populating a phrase in your chart simply press / followed by your phrase prompt.

For example:

  • /P - Patient presents with (symptom or complaint)

  • /dob - Date of birth

  • /virtual - This session was conducted via Telehealth using Jane’s Online Appointment feature.

These phrases should cover the key aspects of a typical visit, and can be easily modified to fit each of your patient’s specific needs.

Robin Valadares, a physiotherapist and Jane Ambassador, is a Phrases enthusiast. “It’s a feature that allows me to create consistent, standardized tests, and assessments. It’s such a time-saver and stops me from having to write the same thing over and over again. I cannot overstate how much I love this feature.”

Spend some time perfecting your notes template

Templates really are a case of well begun, half done. As the saying suggests, spending some time making your notes template work for you is one of the best ways to get great documentation, completed a little faster.

Sean’s advice for getting started with AI Scribe, or any new feature, is to use a template that’s already been built for you when possible. “Change is always hard, and as a busy clinician, we want to make sure these steps are super small.”

Jane’s template library has templates for SOAP notes along with many others, from over 20 disciplines, which can be customized to your liking. Use a fellow practitioner’s template as is, or take the pieces that work for you and your patients, and discard what doesn’t. Even just as a point of inspiration, they’re a great start.

Once you’re comfortable, tweak, edit, and change. And if you’d like, share what you’ve learned with your colleagues. “I’ve worked with a bunch of nerdy physios,” jokes Sean, “and put together what’s called the 1.1 Smart MSK Assessment and the 1.1 Smart MSK Follow-up templates.”
