How to Remove or Delete a File from a Patient Profile

There are two ways to remove a file from a patient’s profile: you can unlink it from the chart (while keeping the file in the Files area), or you can delete it from the profile entirely. This guide covers both.

Option 1: Remove a file from the chart

Use this option when you want to unlink a file from a chart entry but keep it in the patient’s Files area

  1. Go to the Patient’s Profile > Files tab
  2. Hover over the file and select the pencil icon
  3. Toggle off Include in Chart
  4. Select Save

What to Expect

Jane removes the file from the chart automatically. The file stays in the patient’s Files area and can still be viewed, downloaded, or re-added to the chart at any time.


Option 2: Delete a file from Files tab

Use this option when you want to permanently remove a file from the patient’s profile entirely.

Instructions

  1. Go to the Patient’s Profile > Files tab
  2. Hover over the file and select the pencil icon.
  3. Select the trash can icon.
  4. Confirm the deletion in the pop-up.

What to Expect

The file is permanently removed from the patient’s profile and cannot be recovered.


Important Notes

  • Only the staff member who originally uploaded a file or the listed Account Owner can remove it from the chart or delete the file completely.
  • Other staff members won’t have access to modify files they didn’t upload.
  • If a different staff member needs to take ownership of a file, the file must be reuploaded by that staff member.
  • Deleting a file is permanent and cannot be undone.
