Instead of building a Clinical Survey questionnaire from scratch, you can use a chart template to get started. Templates are available from the Chart Template Library (shared by the Jane community) or from templates created in your clinic.
Option 1: Adding a Template to a New Survey
- Go to Settings > Forms & Surveys > View Surveys in the Survey section.
- Click New Survey.
- Scroll down to the Questionnaire section and click the grid icon next to Add Item.
- A pop-up menu appears with all available chart parts, clinic-created templates, and templates from the Chart Template Library:
- Templates: Created by staff members in your clinic. Clicking on a template adds it to the questionnaire automatically.
- Template Library: Created by practitioners across all clinics that use Jane. Clicking on a template opens a preview before adding it.
Option 2: Adding a Template to an Existing Survey
- Go to Settings > Forms & Surveys > View Surveys in the Survey section.
- Click View on the Clinical Survey you want to edit.
- Scroll down to the Questionnaire section and click Edit.
- Click the grid icon next to Add Item.
- A pop-up menu appears with all available chart parts, clinic-created templates, and templates from the Chart Template Library:
- Templates: Created by staff members in your clinic. Clicking on a template adds it to the questionnaire automatically.
- Template Library: Created by practitioners across all clinics that use Jane. Clicking on a template opens a preview before adding it.
What to Expect
After adding the template, the individual chart parts that make up the template appear in the Questionnaire section of your Clinical Survey. You can add, remove, or edit any of the chart parts to customize the questionnaire to fit your needs.
📣 Heads up: Once a Clinical Survey has been completed by at least one patient, the questionnaire section is no longer editable.
Important Notes
- Clinical Surveys and Intake Forms share the same template library.
- To edit a chart part within the survey: Hover over that section > click on the three horizontal black dots > select the pencil icon.
- You can continue to modify chart parts after adding a template, as long as no patient has completed the survey.
