How Do Family Members Fill Out Intake Forms Online?

If your clinic has connected your profile to a family member’s profile, you can fill out intake forms on their behalf through your My Account portal. Once submitted, the completed form is available on your family member’s profile for your clinic to review.

📣 Heads up: If you don’t see a pending intake form for your family member, contact your clinic. Your clinic may need to update your account permissions or resend the form.

To fill out a pending intake form for a family member:

  1. Log in to My Account.
  2. Click the Intake Forms tab.
  3. Click Fill Out Intake Form next to the pending form for your family member.
  4. Review the notice confirming you are filling out the form on behalf of your family member.

  1. Complete the form fields and click Submit.

What to Expect

After you submit the form, it appears in the Intake Forms tab of your My Account page alongside any other forms you have filled out for that family member. Note that you can only view forms you’ve completed on a family member’s behalf and not forms they’ve filled out themselves or that another related profile has submitted for them.

Your clinic receives a notification that the form is complete.

Where Pending Intake Forms Appear

When your family member has a pending intake form, it shows up in two places:

  • Immediately after booking the appointment
  • In the Intake Forms tab of your My Account page

If you skip the form after booking, you can return to it at any time through the Intake Forms tab.
