AI Scribe: Template and Chart Prompts

A prompt is a set of instructions that tells AI Scribe how to write your notes. It’s where you can describe the format you want, the sections to include, and any details that are specific to your practice or discipline.

AI Scribe uses two types of prompts and considers both when generating your notes.

  1. Your Global Prompt is where you store instructions that apply to all your charts.
  2. Your template prompts (or one-off chart entry prompts) are where you get more specific. You can use them for details that are unique to a particular treatment type or session.

In this guide, we’ll go over how to customize your template or one-off chart prompts. Feel free to jump forward to a specific section below:

Using AI Scribe in a one-off chart entry

To add your AI Scribe Smart SOAP Note to a single chart, head to your patient’s chart, click New Chart Entry and select Smart SOAP Note.

You can edit the default prompt by clicking on the note’s label.

The text box has a default prompt that you can edit. Try out the prompt snippets to help with formats other than SOAP, like DAP or BIRP, by clicking the blue Show Prompt Snippets text.

Next, you can decide whether you’d like your generated note to be in narrative or point form using the Format dropdown.

Lastly, you can use the Include the last 10 charts checkbox to choose whether AI Scribe should draw from the patient’s last 10 charts that you wrote when it generates your note.

Testing your note in a chart entry

When editing your prompt for a single chart entry, you won’t have access to the Preview your note area that’s available when editing a template.

To see how your new prompt performs, you’ll need to either upload your own audio file or record new audio. If you don’t like the way your summary turned out and want to test a different prompt using the same audio, you’ll need to download the original recording and create a new chart entry.

To download the audio file, click the microphone icon at the top of the chart entry, then select Manage recordings. This will open a panel on the right-hand side of your screen.

From there, click the three dots beside your recording and choose Download.

Next, you can edit your prompt and then re-upload your audio file to generate a new summary using your updated prompt.

💡 Jane tip: Just a reminder to delete any PHI or PII from your device if you’ve downloaded the audio file from your chart.

Using a Smart SOAP Note in a template

Start by heading to your staff profile and selecting Templates under your name.

Next, click the New template button to create a new one or click Edit next to an existing one.

Click the Add Item button and select Smart SOAP Note from the list.

You can edit the default prompt by clicking on the note’s label.

The text box has a default prompt that you can edit. If you’d like some suggested language, try out the prompt snippets by clicking the blue Show Prompt Snippets text.

Next, you can decide whether you’d like your generated note to be in narrative or point form using the Format dropdown.

Lastly, you can use the Include the last 10 charts checkbox to choose whether AI Scribe should draw from the patient’s last 10 charts that you wrote when it generates your note.

Previewing your note in a template

Before saving your note, you have the option to preview it using one of our sample transcripts.

When you’re happy with your prompt, click Save.

Using another staff member’s template

From the templates area in your staff profile, you can view templates made by other staff members at your clinic.

If you see one you like, we recommend duplicating it so you have your own copy. This way, if the author of the template edits it, their edits won’t affect your copy.

Alternatively, you can just click the toggle next to the template to add it to your list. If the author makes changes to it, it will update for you as well.
