Do I Need Jane's Business Information for the CMS-855B?

Do I need Jane's business name, tax ID, or address for Section 4C of the CMS-855B Medicare enrollment form?

Answer

No, do not enter Jane's legal business name, Jane's Tax Identification Number, Jane's corporate office address, or a Jane server or data center address.

🩵 Jane tip: Jane provides the electronic platform your practice uses to create, maintain, and access records, but your practice remains responsible for those records and for retrieving them when requested. Jane's underlying hosting infrastructure is not a practice location and is not a location that clinic staff or a Medicare reviewer would visit to access your records.

Section 4C (Electronic Storage) of the CMS-855B asks for information about your own practice, not about Jane. When you select Yes to storing patient medical records electronically, enter your practice's own details:

  • Legal business name: your practice's legal name as reported to the IRS
  • Tax Identification Number: your practice's EIN or other TIN used for Medicare enrollment
  • Address: the practice location associated with your Medicare enrollment where authorized staff can access and retrieve records

The legal business name and Tax Identification Number should be consistent with your practice's IRS documentation and Medicare enrollment information.
