The Patient’s Name

What’s in the box?

Forgive us for this one, but in the pursuit of being thorough: this box includes the patient’s full name. It will be entered as Last Name, followed by First Name, and then Middle Initial (if applicable).

Where does this info live in Jane?

The patient’s first, last, and middle names are set within the Patient Profile. To adjust the patient’s name, head to their profile and click Edit/Settings.

Note: If a middle name is entered, Jane will only use the first letter (initial) on the CMS1500.

Is this required?

In Jane, on the CMS1500, and for an EDI:

The patient’s last and first name are required fields. The patient’s middle initial is optional.


What about the EDI file?

Although the patient’s first and last name are always required, where this information is sent in the EDI is situational:

Option 1: Patient is the Subscriber

If the patient is also the Subscriber (the patient is the one who holds the Insurance Policy), then Loop 2000C - Patient is not sent in the EDI. Thus, the patient name is sent in Loop 2000B - Subscriber.

  • The patient’s last name is sent in Loop 2010BA - Subscriber Info, Segment NM103
  • The patient’s first name is sent in Loop 2010BA - Subscriber Info, Segment NM104
  • The patient’s middle name (full name, not initial) is sent in Loop 2010BA - Subscriber Info, Segment NM105

Option 1: Patient is NOT the Subscriber

If the patient is not the subscriber (i.e. their spouse is the Policyholder and they are covered under that Policy), then the patient’s name is sent in Loop 2000C - Patient.

  • The patient’s last name is sent in Loop 2010CA - Patient Info, Segment NM103
  • The patient’s first name is sent in Loop 2010CA - Patient Info, Segment NM104
  • The patient’s middle name (full name, not initial) **is sent in **Loop 2010CA - Patient Info, Segment NM105
